Safety Reporting

IN CASE OF EMERGENCY This form should be filled out LAST. It is not a substitute for contacting emergency services, search and rescue, air traffic control, or maintenance.

Notice The SRS (Safety Reporting System) should not be used to avoid confrontation with, seek punitive action against, or take similar action against other students, instructors, renters, or staff. The SRS is designed to allow anyone to anonymously report safety concerns to be addressed and prevented in the future.

Reporting safety concerns will not result in punitive action except where required by law. This is optionally anonymous. We ask that you provide your name or email, however it is not required. We will not trace anonymous reports. Reporting intentional mistakes does not mitigate punitive action, this reporting system is built for reporting mistakes that could or have resulted in a dangerous flight operation. Reporting runway incursions, critical mistakes, and other errors are best for us. This does NOT replace a NASA report or other similar federal reports. Please fill out the report to the best of your knowledge, and include as many details as possible.
